Paano magpalit ng pangalan sa NSO
Ako po Joy R. Racoma, 23 years old. Ano pwede kung gawin kasi po simula pagkabata Jennelyn R. Racoma ang dala dala kong pangalan hanggang ngayon pati sa mga papers ko ID’s at iba pang dokumento. 5 years ago kumuha ako ng NSO/PSA nakakuha naman ako pero LATE REGISTERED then after 5 years nag-asikaso ako ng papers kasi kukuha ako ng passport at may planong mag abroad. Nag order ako online ng NSO/PSA ng dumating sakin iba na ang spelling at surname. Joy D. Romulo ito ang surname ng Nanay ko nung dalaga pa siya. Ang kwento saken ng magulang ko 2nd family ako ng Nanay at dati bawal gamitin ang surname ng Tatay ko kasi hindi sila kasal. Para magamit ko ang surname ng Tatay ini-register ako sa ibang bayan at kasal ang magulang ko sa NSO/PSA ko. Gusto ko pong gamitin ang pangalang nakasanayan ko simula pagkabata. Maari po ba yun? Iniisip ko baka magkaproblema ako sa pag kuha ng passport kasi twice akong naregister. Ano po bang dapat kong gawin? Sana po ay matulungan nyo ako sa aking problema. Inaasahan ko ang inyong sagot. I can give you information: puedeng itama ang mga entries sa NSO basta't ito ay clerical or typographical errors lamang.
Kung halimbawa ay dalawa ang registration mo, puede ka ring magtanong sa NSO/PSA kung ano ang puede mong gawin. Iyon nga lang kasi, fraudulent ang registration mo, ito ay falsified entries. So, baka makasuhan ang mga mapapatunayang nag-falsify nito.
