1. Dahil sa ang bata ay anak ng ka-live-in mo, at hindi kayo kasal, ang bata ay dapat ipangalan sa iyo -- sa maiden name mo.
2. Kung ang batang iluluwal mo ay tinatanaw ng ka-live-in mo na kanyang sariling anak, puede niyang prmahan ang form sa likod ng birth certificate. Ito ang Affidavit of Recognition and Authority to Use Surname.
Kung pipirmahan ito ng ka-live-in partner mo, puedeng gamitin ng bata ang last name ng ka-live-in mo. It is the right of the child to use the surname of the father who recognizes him as his own child.
