Madali lamang iyan.

Sa liko kasi ng Report of Birth at Certificate of Live Birth, mayroong Affidavit of Acknowledgment. Basta't fill-up-an mo lang ang Affidavit of Acknowledgment.

The best is to talk to a lawyer before you leave the Philippines for abroad.
