Pati pirma wala. Ako lang nakalagay sa birth certificate ng bata. At ngayon gusto namin ilipat sa apilyedo ng tatay niya ano po gagawin namin? Ililipat sana namin ang apelyido ng ank ko sa apelyido ng tatay niya.

Mas mabuting puntahan nalang po ninyo ang Civil Registrar kung saan nakarehistro ang Certificate of Live Birth po. Sabihin po ninyo ang intention ng ama na iacknowledge ang bata bilang anak po niya. May mga requirements po silang dapat ninyong i comply. Isa na po yan ang mag execute ng Affidavit of Recognition/Acknowledgment ang tatay at mag execute din ng Affidavit to Use the Surname ang mother also. Since illegitimate pa ang status ng bata mas mabuting magpakasal na lang din po kayo para magiging legitimate na po ang status ng bata po. Needed po ang appearance mother at father ng bata sa Civil Registrar sa pagprocess nito.
