Ang Birth Certificate, Marriage Certificate, CENOMAR at Death Certificate ay pwedeng mai-apply sa pamamagitan ng forms. At ito ay pwedeng makukuha sa National Statistics Office. Ngunit, ito ay kinokontrol narin. Tig-isang kopya lang ang allowed. Maliban na lang kung isa kang Liaison Officer na naitalaga sa NSO – walang control pag ganito ang set-up.

Meron din tayong Serbilis Outlet, kaso Serbilis nga pag wala ka nang problema, kaso pampatagal pag nagkataon.
Mga kailangang i-fill up upang makakuha ng Birth Certificate:
1. Complete name of the child (first, middle, last)
2. Complete name of the father
3. Complete maiden name of the mother
4. Date of birth (month, day, year)
5. Place of birth (city/municipality, province)
6. Whether or not registered late. (If registered late, please state the year when it was registered.
7. Complete name and address of the requesting party
8. Relationship to the child
9. Number of copies needed
10. Purpose of the certification
NOTE: ISSUANCE OF CERTIFICATION OF BIRTH IS SUBJECT TO THE CONFIDENTIALITY CLAUSE OF ARTICLE 7 OF THE CHILD AND YOUTH WELFARE CODE WHICH PROVIDES:
Art. 7. Nondisclosure of birth records. The records of a person's birth shall be kept strictly confidential and no information relating thereto shall be issued except on the request of any of the following:
1. The person himself, or any person authorized by him;
2. His spouse, his parent or parents, his direct descendants, or the guardian or institution legally incharge of him, if he is a minor;
3. The court or proper public official whenever absolutely necessary in administrative, judicial or other official proceedings to determine the identity of the child's parents or other circumstances surrounding his birth; and
4. In case of the person's death, the nearest of kin.
Any person violating the prohibition shall suffer the penalty of imprisonment of at least two months or a fine in an amount not exceeding five hundred pesos, or both, in the discretion of the court.
Releasing
Normally, tig-isang oras lang, pag minsan hanggang maghapon, kinabukasan, pag di ka masuwerte aabot yan hanggang one month, lalong lalo na pag may problema yung First Name, di kaya ay Middle Name, o kaya ang Last Name kung saan kailangan mo ng Supplemental Report.
In total, depende rin yan sa dami na kumukuha ng BC, MC, CENOMAR, at DC (Death Certificate) sa Serbilis Center.
May problema ba kayo na naka-base sa maling First Name, Middle Name, at saka Last Name? Ang pinaka-mabilis ay magreport sa Local Register kung saan ka pinanganak. Oo, ganun ka-simply, kaso pag nasa Manila ka na, at pinanganak ka sa ibang lugar, at may problema ka na, kailangan mong bumalik dun kung saan ka pinanganak, o pwedeng magdala ng sulat as authorization kasama ang ID. Yun ay para asikasuhin ang problema.
Mga kailangan:
1. Magdala ng supporting documents (school records, baptismal certificate, at iba pa) para sa pagpatutuo na ikaw, ay may problema gaya ng First Name, Middle Name, etc .. at ikaw ay pinanganak sa lugar na iyon. At kailangan nila LCRO(Local Civil Registry Office) na mag-submit ng Supplemental Report dun sa NSO.
Pag may problema ka sa BC, MC, DC, CENOMAR, pumunta ka lang sa LCRO. Yan lang ang umpisa, kagaya ng iba pang nag-proprovide ng article, kailangan mong susundin ang mga steps na i-proprovide ng LCRO. At para sigurado, maglaan ka din ng isang libo (1,000.00) para mai-process ang mga request.
At halos pare-parehas ang steps na ginawa pag meron kang problem sa DC, MC, CENOMAR, at saka sa BC. :)
